Cognitive / Concussion Therapy

For over 10 years, Peak & Balance Centers of America has been involved in the assessment and management/rehabilitation of mild traumatic brain injury (mTBI) and concussion injuries. We emphasize the comprehensive evaluation process, using objective testing methods to determine the magnitude and location of the injury. Peak adheres to the currently accepted guidelines for removal from participation in activity in accord with the:

  • Laws of the State of Illinois
  • Medical guidelines of the American Academy of Neurology (AAN)
  • Medical guidelines of the American Academy of Sports Medicine (AAOSM)
  • Medical guidelines of the American Physical Therapy Association (APTA)

While we emphasize the vestibular components of the injury, we also assess and manage the cognitive, visual, and other associated manifestations. To assess the various presentations of both sports-related and non-sports-related mTBI and concussive injuries, we use: 

  • Infra-red Oculography
  • Vestibular Evoked Myogenic Potential (VEMP)
  • Vorteq (VOR reflex)
  • Dynamic Visual Acuity (DVAT)
  • Gaze Stability Test (GST)
  • EVOKE Cognition Assessment
  • King-Devick Test
  • ImPact Test
  • Computerized Dynamic Posturography

Sports-related mTBI and Concussions

For sports teams that we are involved with, we provide pre-season screenings of athletes to establish baseline functions for eye focus, cognition, and balance, using: 

  • SCAT3
  • King-Devick
  • NeuroTracker
  • DVA
  • IR Oculography

After an injury, the tests are repeated for comparison to the baseline. In cases where the pre-season screen has not been performed, we use normative, aged-matched data for comparison. In addition, we administer the more detailed in-office tests listed above to measure and objectify the magnitude of injury. 

From the symptoms reported, along with the objective testing, we create a protocol for an athlete's removal from participation, rehabilitation, and return to participation.

In-office and home-based exercise programs are used in the rehabilitation process, including: 

  • DVA and VOR Reflex Exercises
  • Reaction Times Exercises
  • Balance and Equilibrium Exercises
  • Cognitive Rehabilitation (BrainWare)
  • Vision Therapy Techniques

These test results and rehabilitation improvements are relayed to the athlete’s personal MD and team medical staff for their participation in the decision-making processes, including return to participation. Peak serves as liaison between the athlete, the team, and the school (if pertinent) for removal and return to participation (homework, tests, attending class, etc.) for “return to learn.”

Concussion Research

Peak has participated in various research projects involving sports concussion, including in-helmet sensor technology for tracking the number and magnitude of impacts and comparing that data to objective post-injury testing. Other ongoing topics are being analyzed: visual distortion due to poor visual acuity between the two eyes; the use of virtual reality as a rehabilitation tool; loss of balance and equilibrium and disorientation; cognition changes with memory, reaction times, etc.; and, more recently, the genetic components of injury predisposition and recovery rates with various genetic components detection.

Non-sports-related mTBI and Concussions

When assessing non-sports related mTBI and concussion injuries, Peak follows a process similar to that used for athletic groups. Based on our assessment, we will create testing and rehabilitation protocols for return to work and activities of daily living (ADLs). We also offer a driving evaluation and assessment, and we will perform work capacity evaluations as needed.

This comprehensive method has afforded Peak a greater than 95% success rate in detection of injury, proper graded and gradual rehabilitation process, life management decisions, and return to participation for the sports and non-sports-related populations.
